Soutenir l’inclusion dans l’essai clinique CO.LOMB : retour d’expérience sur la formation des internes à la recherche et la revue des dossiers médicaux en soins primaires

Introduction : la lombalgie commune constitue un problème majeur de santé publique. Malgré l’intérêt démontré d’une prise en charge coordonnée, la recherche clinique en soins primaires demeure peu développée et confrontée à des difficultés récurrentes de recrutement. L’étude CO.LOMB, essai multicentrique randomisé en grappes coordonné par le DUMG, évalue l’impact d’une prise en charge pluriprofessionnelle coordonnée sur le handicap fonctionnel des patients lombalgiques.

Objectifs : les objectifs de ce travail étaient de former les internes de médecine générale à la recherche clinique et aux modalités de l’étude CO.LOMB, et de réaliser des revues de dossiers médicaux pour identifier des patients potentiellement éligibles.

Expérience : cette thèse prend la forme d’un retour d’expérience. Les actions menées reposaient sur trois axes : une phase préparatoire comprenant la rencontre des acteurs de l’étude et la formation des thésards aux bonnes pratiques cliniques, la formation et l’animation d’une communauté d’internes, et la réalisation de revues systématiques de dossiers médicaux dans les cabinets investigateurs. Au total, 31 internes ont été formés au cours de cinq sessions organisées en 2025. Les questionnaires ont mis en évidence un intérêt globalement modéré pour la recherche clinique ainsi que plusieurs freins à l’engagement, notamment le manque de temps. Par ailleurs, des interventions ont été réalisées sur 10 sites investigateurs de la subdivision angevine, représentant plus de 54 heures de travail. Les revues de dossiers ont permis le screening de 620 dossiers médicaux et le repérage de 119 patients potentiellement éligibles, dont 54 directement éligibles et 65 éligibles sous réserve d’une nouvelle consultation dans les 12 mois.

Caractère original : l’originalité de ce travail repose sur l’implication directe d’internes de médecine générale dans le soutien opérationnel d’un essai clinique interventionnel en soins primaires, notamment à travers la réalisation de revues de dossiers médicaux dans les cabinets investigateurs. Cette démarche est peu décrite dans la littérature.

Enseignements tirés de l’expérience : cette expérience a mis en évidence l’hétérogénéité des logiciels métiers, les contraintes organisationnelles pesant sur les médecins investigateurs et la nécessité de développer des outils et ressources dédiés à la recherche en soins primaires. Elle a également souligné l’intérêt de l’implication des internes comme levier d’acculturation à la recherche.

Conclusion : ce travail a permis de sensibiliser les internes à la recherche clinique en soins primaires et de soutenir le recrutement de l’étude CO.LOMB. Il souligne la nécessité de renforcer les infrastructures, les outils et l’accompagnement dédiés au développement de la recherche en médecine générale.

Introduction : non-specific low back pain is a major public health issue. Despite the demonstrated benefits of coordinated care, clinical research in primary care remains underdeveloped and faces persistent recruitment challenges. The CO.LOMB study, a multicenter cluster-randomized trial coordinated by the Department of General Practice, evaluates the impact of coordinated multidisciplinary management on functional disability in patients with non-specific low back pain.

Objectives : the objectives of this work were to train general practice residents in clinical research and in the procedures of the CO.LOMB study, and to conduct medical record reviews to identify potentially eligible patients.

Experience : this thesis takes the form of an experience report. The activities were structured around three main components : a preparatory phase involving meetings with study stakeholders and training of the doctoral candidates in Good Clinical Practice; the training and facilitation of a community of residents; and the conduct of systematic medical record reviews within participating investigator practices. A total of 31 general practice residents were trained during five training sessions held in 2025. Post-training questionnaires revealed a generally moderate level of interest in clinical research and identified several barriers to engagement, particularly lack of time. In addition, interventions were carried out across 10 investigator sites within the Angers academic region, representing more than 54 hours of work. Medical record reviews enabled the screening of 620 patient records and the identification of 119 potentially eligible patients, including 54 immediately eligible patients and 65 who could become eligible following a new consultation within the next 12 months.

Originality : the originality of this work lies in the direct involvement of general practice residents in the operational support of an interventional clinical trial conducted in primary care, particularly through systematic medical record reviews within investigator practices. Such an approach has been scarcely described in the literature.

Lessons learned from the experience : this experience highlighted the heterogeneity of electronic medical record systems, the organizational constraints faced by investigator physicians, and the need for dedicated tools and resources to support primary care research. It also underscored the value of resident involvement as a means of fostering a research culture within general practice.

Conclusion : this work helped raise awareness of clinical research among general practice residents and supported recruitment for the CO.LOMB study. It highlights the need to strengthen infrastructure, tools, and support mechanisms dedicated to the development of research in general practice.
