Au cours de notre stage, nous avons initié un travail de recherche sous la forme d'une étude de cas, avec M. E, sujet entrant en hémodialyse de formation au centre de dialyse ECHO à Angers. Nous avons pu nous étonner du positionnement original de ce monsieur, qui, contrairement aux autres patients, semblait énoncer une amélioration de son bien être dans le cadre de la dialyse. Notamment en développant sur sa machine dialyse un discours tout à fait étonnant, nous suggérant alors la piste d'un lien privilégié avec celle-ci. Nous chercherons à comprendre ce qui dans la rencontre de M. E, avec sa machine, et l'institution, concourt à participer à la remise en sens de son histoire, après le traumatisme de son entrée à la retraite. Nous proposerons alors l'analyse suivante : M. E lors de son entrée en dialyse, formera alliance avec la machine, qui sera ensuite repensée, tandis que progressivement, un tiers « institution » viendra proposer un cadre maternant et proposera une stabilité dans la relation de M. E avec sa machine. Celui-ci pourra alors accéder à l'homéostasie psychique permettant une certaine forme de continuité d'existence identitaire interrompue par son passage à la retraite.
During our placement we began a research project which took the form of a study case of Mr E, a patient at the center for dialysis, in Angers. Mr E's original attitude took us by surprise as he, contrary to other patients, declared an improvement in his well being. In particular during his treatment on the dialysis machine we noticed, through we discussed, the emergence of a special link with the machine. We are trying to understand which elements of Mr E's encounter with the dialysis machine and the institution itself contribute towards finding meaning, after his traumatic transition into retirement. We propose the following analysis: when Mr E begins his dialysis treatment he will form a bond with his machine, this bond which will change. During this time a third party, the institution, will offer a maternal, safe environment as well as relational stability between Mr E and his dialysis machine. In this way he will be able to acquire a “psychological homeostasis” which will allow him to maintain his own identity that was interrupted by his retirement.
