Introduction : recommendations regarding the prescription of statins for primary prevention are debatable and sometimes inconsistent. While the use of a score to assess the 10-year risk of a cardiovascular event is relatively widely accepted, the target LDL level—which is set ever lower, leading to an ever increasing number of patients requiring treatment—remains a subject of debate. The main objective of this thesis was to identify the factors influencing the initiation of statin therapy among primary care physicians, who play a key role in primary prevention.
Materials and Methods : this was a qualitative study using semi-structured individual interviews, conducted until data saturation was reached between October 2025 and March 2026. The study was designed to meet as many of the COREQ framework’s quality criteria as possible. The analysis was conducted using a grounded theory approach.
Results : a total of ten general practitioners were interviewed. They place great importance on patients’ lifestyles and risk factors. They describe a personalized, non-urgent approach to care, in which shared medical decision-making plays a central role. This process is influenced by the perceptions of both the physician and the patient. There are multiple criteria for initiating statin therapy. The medical decision may be guided by the results of additional tests to screen for vascular complications.
Conclusion : this study demonstrated the heterogeneity of general practitioners’ practices regarding the initiation of statins for primary prevention, a consequence of recommendations that lack clarity and are difficult to apply. It is generally the culmination of a lengthy process based on shared medical decision-making.
