Le suivi psychothérapeutique de Mme A, 25 ans, initialement hospitalisée en psychiatrie, a permis d’interroger les conséquences d’un environnement précoce carentiel et maltraitant sur la construction psychique, narcissique et sur l’enveloppe psychique. La méthodologie et le lien théorico-clinique ont montré des processus témoignant d’un traumatisme psychique non élaboré, fragilisant les capacités de symbolisation et la contenance du Moi-peau. Cette organisation psychique semble s’accompagner d’un vécu de vide et d’angoisses de perte d’objet, impactant la constitution identitaire, ainsi que les représentations de soi et des autres. Les conduites non symbolisées apparaissent comme des modalités de traitement de ces angoisses afin d’éviter la réactivation de la blessure narcissique et d’une souffrance psychique menaçant le Moi d’effondrement. Ce travail a permis de proposer des pistes d’accompagnement et souligne la singularité du fonctionnement psychique de Mme A, qui ne peut être généralisé.
The psychotherapy of Ms. A., a 25-year-old woman initially hospitalized in adult psychiatry, explored the consequences of an early neglectful and abusive environment on psychic construction, narcissistic functioning, and the psychic envelope. The methodology and the articulation between theory and clinical material highlighted processes reflecting unprocessed psychic trauma, weakening symbolization capacities and the containing function of the Skin-Ego. This psychic organization is associated with feelings of emptiness and object-loss anxieties, affecting identity construction as well as self and object representations. From this perspective, non-symbolized behaviours appear as attempts to manage them to avoid the reactivation of narcissistic injury and unbearable psychic suffering threatening ego collapse. This work offers avenues for clinical support concerning subjects with this type of difficulty, while emphasizing the non-generalizable nature of Ms. A.’s psychic functioning.
