Objectif : l'objectif de cette étude était d'analyser l'association entre presbyvestibulopathie et altération des paramètres de la marche sur tapis de marche GAITRite® chez des adultes de 75 ans et plus, présentant des troubles de la marche ou une instabilité.
Méthode : une étude observationnelle monocentrique menée en France au CHU d'Angers, a inclus 55 patients ayant eu un examen vestibulaire standardisé, nécessaire pour appliquer les critères de la Barany Society pour le diagnostic de presbyvestibulopathie, ainsi qu'une analyse de la marche sur le tapis de marche GAITRite® dans différentes conditions. (rythme de marche habituel, marche rapide et marche en double tâche).
Résultats : parmi 55 patients (moyenne 84,2 ± 5,2 ans ; 65,5 % de femmes), 23 présentaient une presbyvestibulopathie (42 %). L'analyse univariée a indiqué une longueur de foulée plus courte chez les patients atteints de presbyvestibulopathie au rythme de marche habituel (91,1 cm [76,9-105,9] versus 99,6 cm [85,6-114,9], p=0,044) et en condition de double tâche (91,3 cm [73,8-101,7] versus 103,6 cm [89,0-118,9] p=0,048) et une base d’appui plus large en condition de double tâche (11,16 cm [8,30-13,83] versus 9,94 cm [7,08-11,46], p=0,031), par rapport aux patients sans presbyvestibulopathie. Ces résultats ont été confirmés par analyse multivariée après ajustement sur l'âge, le sexe, l'indice de masse corporelle et le score du Mini-Mental State Examination (longueur de foulée : RC=0,96 [0,932 ; 0,997], p=0,032 ; base d'appui : RC=1,20 [1,00- 1,43], p= 0,046 en condition de double tâche).
Conclusion : les paramètres de marche sont associés à la presbyvestibulopathie chez les personnes âgées. Le dépistage de la presbyvestibulopathie chez les patients présentant des troubles de la marche et de l'équilibre peut permettre d'apporter une assistance adaptée et ainsi réduire le risque de chute.

Results : among 55 patients (mean, 84.2±5.2years; 65.5% women), 23 had presbyvestibulopathy (42%). The univariate analysis indicated a shorter stride length in patients with presbyvestibulopathy at usual walking pace (91.1cm [76.9-105.9] versus 99.6cm [85.6-114.9], p=0,044) and under dual tasking condition (91.3 cm [73.8-101.7] versus 103.6 cm [89.0-118.9] p=0,048) and a higher support base under dual tasking condition (11.16 cm [8.30-13.83] versus 9.94 cm [7.08-11.46], p=0,031), compared to patients without presbyvestibulopathy. These results were confirmed by multivariate analysis after adjustment for age, sex, body mass index and Mini-Mental State Examination score (stride length: RC=0.96 [0.932;0.997], p=0.032; support base: RC=1.20 [1.00-1.43], p= 0,046 under dual tasking condition).
Conclusions : Gait parameters are associated with presbyvestibulopathy in older adults. Screening for presbyvestibulopathy in patients with gait and balance disorders may help to provide appropriate assistance and thus reduce fall risk.
