| Résumé | Introduction : depuis le début des années 2000, les missions du pharmacien hospitalier ont grandement évolué, notamment avec la formalisation des missions de pharmacie clinique. La nouvelle gradation des activités ambulatoires détaille la réalisation de consultations pluridisciplinaires (CP), en y intégrant le pharmacien. Objectif : nous avons souhaité développer les CP pour le suivi des patients bénéficiant d’une transplantation hépatique au CHRU de Tours. Matériel et Méthode : un groupe de réflexion de professionnels spécialisés a été formé. Après identification des complications survenant chez ces patients et de leurs facteurs de risque, une fiche projet détaillant l’organisation du programme, les participants et ses attendues a été rédigée. Cette fiche a été soumise aux différentes instances hospitalières. De même une évaluation financière a été réalisée. Résultats : les consultations auront lieu mensuellement la première année suivant la transplantation puis une fois par an, à la date anniversaire de la greffe. Les intervenants se décomposent en 2 catégories. Le trio socle, composé de l’hépatologue, du pharmacien et de l’infirmier de coordination, qui interviendra à chaque CP ; et les intervenants ponctuels qui participeront de façon adaptée aux besoins des patients. Enfin, l’évaluation économique de ce projet est positive pour l’établissement. Conclusion : la fiche projet a été validée par le groupe de réflexion et les pôles impliqués. La révision de la gradation des activités ambulatoires permet la mise en place d’une telle activité. Elle a notamment permis de financer des professionnels dont les interventions individuelles ne donnent pas lieu à facturation. |
| Résumé en anglais | Introduction : hospital pharmacist missions have highly changed since the 2000s. Particularly, the new mission of clinical pharmacy is a corner in profession progression. The new ambulatory activity graduations introduce the requirements for the development of multidisciplinary consultations (MC), introducing the pharmacist. Objective : we aimed to develop the activity of MC for the liver transplant recipient follow-up in CHRU de Tours. Material and Method : A focus group has been formed with specialists’ caregivers. After introducing the clinical complications and risk factors among this population, the project sheet related the project organization and the impact on the CHRU. This document, with an economic and organizational evaluation of the project, has been presented for submission to the CHRU instances. Results : MC occurs monthly for the first year after transplantation, then at each transplantation anniversary consultation. Two categories of caregivers are defined. The first one performs cares at every patient consultation and is composed with the physician, the pharmacist, and the nurse. The second concern all ad hoc stakeholders with adapted interventions on the patient needs. This organization shows a positive economic balance. Conclusion : the project sheet has been approved by the focus group. This activity is now economically possible in France, thanks to the new ambulatory activity graduations. It allows medical and paramedical professionals implementation in the patient follow-up that were not allowed to bill their interventions. |
