| Résumé | L'arthroplastie du genou pour cause dégénérative est l'une des arthroplasties les plus fréquemment pratiquées dans le monde et propose un remplacement uni-, bi- ou tri-compartimental du genou. Il existe différents types d'arthroplasties, des plus mobiles aux plus contraintes : non contraintes (à glissement), semi-contraintes et contraintes. L'arthroplastie tri-compartimentale non contrainte peut être associée ou non au sacrifice du ligament croisé postérieur, qui sera compensé par différents systèmes : cage de postéro-stabilisation, troisième condyle ou prothèses ultra-congruentes. Toutes ces arthroplasties sont exposées à des complications communes et spécifiques, notamment en fonction du nombre de compartiments remplacés. La radiographie standard reste l'examen de première intention pour le suivi prothétique ou lorsqu'une complication est suspectée. Les autres modalités (échographie, tomodensitométrie, imagerie par résonance magnétique) ont chacune leurs avantages/inconvénients et doivent être précisément indiquées. Les complications mécaniques, septiques ou liées à l'insert de polyéthylène, les synoviopathies, les tendinopathies, les neuropathies, les vasculopathies et les ossifications hétérotopiques sont communes à tous les types d'arthroplasties. L'usure du compartiment controlatéral est une complication spécifique des arthroplasties unicompartimentales (1). Avec l'augmentation du nombre d'arthroplasties, il est essentiel pour le radiologue et le chirurgien orthopédique de maîtriser les critères d'implantation et les complications de tous les types d'arthroplasties suivants les différents types d'imagerie nécessaires à leur diagnostic. Ce travail propose de synthétiser les aspects normaux et pathologiques des différents types d'arthroplasties afin d'obtenir un diagnostic précoce des éventuelles complications. |
| Résumé en anglais | Knee arthroplasty for degenerative causes is one of the most frequently performed arthroplasties worldwide and offers single, bi- or tri-compartmental knee replacement. Various types of arthroplasties exist, from the most mobile to the most constrained: unconstrained arthroplasty, semi-constrained arthroplasty, constrained arthroplasty. The unconstrained tri-compartmental arthroplasty may or not may be associated with the posterior cruciate ligament sacrifice, which will be compensated by various systems: posterior-stabilized box, third condyle, or ultra-congruent arthroplasty. All these arthroplasties are exposed to common and specific complications, notably depending on the number of compartments replaced. Standard radiography remains the first - line examination for prosthetic follow-up or when a complication is suspected. The other modalities (ultrasound, computed tomography, magnetic resonance imaging) each have their advantages/disadvantages and must be precisely indicated. Mechanical, septic or polyethylene insert-related complications, synoviopathies, tendinopathies, neuropathies, vasculopathies and heterotopic ossifications are common to all types of arthroplasties. Controlateral compartment wear is a specific complication of unicompartmental arthroplasties (1). With the increasing number of arthroplasties, it is essential for the radiologist and the orthopaedic surgeon to master the implantation criteria and the complications of all types of arthroplasties according to the different types of imaging necessary for their diagnosis.
