Résumé en anglais | Consultation-liaison psychiatry in general hospitals has been developed since the 20th century. Few studies have been carried out to collect the opinions of requesting physicians on its interventions in the care process. The objective of this thesis is to carry out an inventory of the perception of doctors seeking advice on liaison psychiatry at the Angers University Hospital. A questionnaire was sent by email from July to October 2020 to all doctors who can call on the liaison psychiatry at Angers University Hospital. 169 responses from 33 departments were taken into account. 95.9% of doctors ask for a psychiatric opinion in front of a patient with a psychiatric disorder. The main difficulties in dealing with patients presenting psychiatric symptoms are the nature and degree of urgency of the psychiatric disorder, the indication for transfer to a specialized unit and the refusal of the patient or the family. The least unanimous indications and interventions are announcements of a serious diagnosis, acute confusional state, refusals of care, social problems, the alternative to hospitalization in somatic care, the response to a request from the patient's family, resolution of a conflict between the patient and the healthcare team and finally participation in a multidisciplinary consultation meeting. This inventory made it possible to highlight the demands and difficulties currently being felt by the doctors at the Angers University Hospital. Training and information wishes on psychiatric disorders are mentioned there. The difficulties mentioned are the management of patients with pathologies intertwined between two disciplines such as neuropsychiatry or gerontopsychiatry as well as the difficulties of diagnosis and hospitalization of psychiatric patients. Improving the knowledge and communication of consultation-liaison psychiatry could allow better collaboration between different practitioners for patient care. |
