Intérêt d'une check-list dans la prise en charge de l'hémorragie du post-partum

L’hémorragie du post-partum (HPP) survient dans 11,6 % des accouchements, entraînant de nombreuses complications maternelles. La performance des équipes obstétricales dans la gestion de cette urgence est donc essentielle.

 Objectif : examiner l'impact d'une check-list spécifique de la gestion de l'hémorragie du post-partum sur la performance des équipes obstétricales. 

Matériel et méthode : étude quantitative, comparative et prospective, réalisée en simple aveugle au Centre de Simulation du CHU d'Angers, en janvier 2024 et 2025. 

Résultats : Le score de performance moyen a été de 18/24 items pour les équipes sans check-list, contre 23/24 items pour celles utilisant la check-list (p=0,001). Le temps moyen de prise en charge était de 1255 secondes pour les groupes sans check-list contre 1052,75 secondes pour ceux avec check-list (p=0,006). Cet outils a permis une amélioration de la satisfaction globale du groupe (p=0,039), il n’a en revanche été observé aucun impact significatif sur la satisfaction individuelle des participants (p=0,34) ni sur leur niveau de stress (p=0,49). 

Conclusion : cette étude met en lumière l'intérêt de l'utilisation d'une check-list pour la gestion de l'hémorragie du post-partum. Une étude in vivo est proposée, afin d’évaluer ses bénéfices dans des conditions réelles. Par ailleurs, l’impact psychologique de son énonciation à voix haute sur la patiente mériterait également d’être étudié. Postpartum hemorrhage (PPH) occurs in 11.6% of deliveries, leading to numerous maternal complications. The performance of obstetric teams in the management of this emergency is therefore essential.

 Objective : to examine the impact of a specific checklist for the management of postpartum hemorrhage on the performance of obstetric teams. 

Material and method : quantitative, comparative and prospective study, carried out in single blind at the Simulation Center of the Angers University Hospital, in January 2024 and 2025.

 Results : the mean performance score was 18/24 items for teams without a checklist, versus 23/24 items for those using the checklist (p=0.001). The mean support time was 1255 seconds for groups without a checklist versus 1052.75 seconds for those with a checklist (p=0.006). This tool allowed an improvement in the overall satisfaction of the group (p=0.039), however, no significant impact was observed on the individual satisfaction of the participants (p=0.34) or on their stress level (p=0.49). 

Conclusion : this study highlights the interest of using a checklist for the management of postpartum hemorrhage. An in vivo study is proposed to evaluate its benefits in real conditions.
